Initial Multicenter Experience Using a Novel Articulating Through-the-Scope Traction Device for Endoscopic Submucosal Dissection

چکیده

Background: A single-operator, articulating, through-the-scope (TTS) traction device was recently developed to facilitate endoscopic submucosal dissection (ESD). Clinical data on the performance of this is limited. We report an initial multicenter experience ESD using articulating TTS device. Methods: Retrospective analysis all consecutive patients who underwent (T-ESD) at 5 centers between August 2021 and December 2022. Endpoints included: rates en-bloc resection, R0 curative adverse events. Results: Thirty-six (median age 64.8 years; 47.2% women) lesion size 40 mm; interquartile range [IRQ]: 27.5-67.5) for lesions in esophagus (n=2), stomach (n=8), sigmoid colon (n=6), rectum (n=20). Submucosal fibrosis encountered a third (33.3%). Median time 104.6 minutes (IQR: 65-122). En-bloc, resection were achieved 94.4%, 91.6%, 97.2% respectively. The single patient with non-curative invasive rectal adenocarcinoma surgery. There no cases delayed bleeding or perforation. recurrence surveillance endoscopy (n=20) median 6 months 3.75-6). Conclusion: This demonstrates high excellent safety profile when performing novel Dynamic real-time may lower technical difficulty ESD. Counter Traction Makes Endoscopic Submucosal Dissection Easier

Poor counter traction and poor field of vision make endoscopic submucosal dissection (ESD) difficult. Good counter traction allows dissections to be performed more quickly and safely. Position change, which utilizes gravity, is the simplest method to create a clear field of vision. It is useful especially for esophageal and colon ESD. The second easiest method is clip with line method. Counter ...

Traction device to remove an adenoma in the appendiceal orifice by endoscopic submucosal dissection.

appendiceal orifice by endoscopic submucosal dissection Adenomatous polyps of the appendix are rare [1,2]; in some cases polypectomy is unsuccessful and surgical treatment is required [3]. Appendiceal polypectomy at colonoscopy is carried out extremely rarely [4]. This is the first reported case of an adenoma of the appendix resected using a traction device during endoscopy. A 63-year-old man a...

Is the Endoscopic Grasp-and-Traction Device Useful for Endoscopic Submucosal Dissection in Treating Early Gastric Cancer?

has become possible to achieve en bloc and histologically complete resection of gastric tumors with endoscopic submu-cosal dissection (ESD), regardless of size, allowing for the re-section of tumors that would not have been previously possible. 1,2 However, ESD is a challenging technique with a higher complication rate than that of endoscopic mucosal resection. The main difficulty of ESD is the...

Endoscopic submucosal dissection using a novel irrigation hood-knife.

Endoscopic submucosal dissection tech− niques using an insulated−tip knife (IT knife) were introduced several years ago to allow en−bloc resection of early gastric cancers, since histological evaluations are essential in order to assess the risk of lymph−node metastasis [1, 2]. However, the technique requires special skills and involves a prolonged procedural time. We have therefore developed a...

Endoscopic submucosal dissection for small submucosal tumors of the rectum compared with endoscopic submucosal resection with a ligation device

AIM To evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) for small rectal submucosal tumors (SMTs). METHODS Between August 2008 and March 2016, 39 patients were treated with endoscopic submucosal resection with a ligation device (ESMR-L) (n = 21) or ESD (n = 18) for small rectal SMTs in this study. Twenty-five lesions were confirmed by histological evaluation of endos...
